Add STATICIRAM_NOT_MDCT as a workaround for the gcc bug where static functions are not long called. This is currently only an issue on the Nano2G (S5L8701 CPU) which is the only ARM target with enough IRAM for these functions. Also add multiple-inclusion protection for config-tremor.h. All plugins and codecs now compile cleanly for the Nano2G.

Also introduce BOOTFILE_EXT2 define for an alternative firmware file extension and add .ipodx for the Nano 2G (.ipod is for unencrypted images, similar to older ipods, and .ipodx is for encrypted images and include the 2KB hash block and modelname 'nn2x').

Quick fix to allow certain MP3 files to play. If a Xing or Info header didn't properly set both the frame count and byte count, then the bitrate was set to zero, preventing playback. Better to use the bitrate of the first audio frame in this case. (More sanity checking of the frame count should perhaps be done.)

Use cookies for thread identification instead of pointers directly which gives a buffer against wrongly identifying a thread when the slot is recycled (which has been nagging me for awhile). A slot gets 255 uses before it repeats. Everything gets incompatible so a full update is required.
